1. What Is Traditional Liposuction?

Traditional liposuction is a surgical body contouring procedure performed to reduce localised fat deposits that are resistant to diet and exercise, and to reshape the body contour.

It is not a weight-loss operation. The goal is to refine body lines at the patient’s current weight by reducing disproportionate fat accumulations in areas such as the waist, abdomen, hips, thighs and back, and to create a more balanced silhouette.

“Traditional liposuction” refers to the standard technique used for many years, in which fat is removed with a vacuum system. In suitable patients, it can also be combined with other liposuction technologies.

2. In Which Situations Is Liposuction Preferred?

Liposuction is typically considered in the following situations:

• Localised fat excess that does not improve despite diet and exercise
• “Love handles” around the waist, abdominal bulging
• Disproportionate fat deposits on the hips, outer thighs (“saddlebags”), inner knees, back or under the chin
• The desire for smoother body contours under clothing
• As part of a combined plan together with other body contouring surgeries such as tummy tuck, BBL or thigh lift

The aim here is not to “turn the patient into a different body size”, but to make their existing body shape more proportional, balanced and aesthetically pleasing.

3. Who Is a Good Candidate for Liposuction?

Ideal candidates for liposuction are those who:

• Are in generally good health and suitable for surgery
• Do not have a very high body mass index
• Have a primary problem of localised fat excess
• Have reasonably preserved skin elasticity
• Understand that liposuction is a contouring, not a weight-loss, procedure
• Are willing to follow postoperative instructions regarding compression garment use, movement and lifestyle changes

In patients with very high body weight or severe obesity, a weight-loss programme or bariatric surgery may need to be considered first.

In conditions such as uncontrolled diabetes, significant cardiovascular disease, clotting disorders or advanced lung disease, a detailed evaluation is required before any decision is made.

4. Preoperative Evaluation and Planning

Before liposuction:

• Your medical history, current medications, previous surgeries and allergies are thoroughly reviewed.
• Areas with excess fat are examined; skin laxity, cellulite and skin quality are evaluated.
• Which areas can be treated, how much fat can realistically be removed and what degree of shape change can be expected are discussed in detail.

5. How Is Traditional Liposuction Performed?

Traditional liposuction can be performed under local anaesthesia with sedation or under general anaesthesia. The choice depends on the number and size of the areas to be treated.

Marking:

• Before surgery, while the patient is standing, the surgeon marks the areas where fat is most prominent.

Tumescent Solution:

• A special solution is injected into the treatment area. This solution:
  1. Reduces bleeding
  2. Helps control pain
  3. Facilitates easier and smoother fat removal

Fat Removal (Aspiration):

• Thin cannulas (2–4 mm) are inserted under the skin through small incisions (usually only a few millimetres).
• Excess fat tissue is gently suctioned out using a vacuum system in a controlled manner.
• The aim is not to flatten the area completely, but to achieve a natural-looking reduction in volume and a contour that harmonises with the rest of the body.

Closure and Dressing:

• The tiny incisions are usually closed with a few stitches or left as small openings that can heal on their own.
• Elastic bandages are applied and then a special liposuction compression garment is put on.

Liposuction is most often performed in areas such as:

• Abdomen and waist
• Hips and outer thighs
• Inner knees
• Back and bra line
• Under the chin (double chin)

6. Postoperative Process, Stay in Istanbul and Follow-Up Visits

After surgery, the patient is monitored closely for the first few hours. For limited-area liposuction, same-day discharge is often possible, while for multiple areas, an overnight hospital stay may be preferred.

First Days:

• Swelling, bruising, tenderness and a feeling of tightness in the treated areas are normal.
• Painkillers, anti-oedema medications and other prescribed drugs are taken regularly.
• Short, frequent walks are important to support circulation.

7. Compression Garment, Swelling, Bruising and Daily Life

Compression Garment:

• Special compression garments after liposuction help maintain the new shape and reduce swelling.
• They are typically worn for 4–6 weeks, most of the day in the first weeks, then gradually reduced in duration.
• The exact duration and schedule are tailored to the patient at follow-up visits.

Swelling and Bruising:

• Bruising and swelling are usually most noticeable in the first few days.
• Bruises generally resolve significantly within 2–3 weeks, while complete resolution of swelling can take several months.
• Reducing salt intake, drinking plenty of water and taking gentle walks support the healing process.

Daily Life:

• For the first 1–2 weeks, very tight clothing and movements that put direct pressure on treated areas should be avoided.
• Light showers, personal care and simple household activities are reintroduced gradually, as permitted by your doctor.

8. Possible Risks and Complications of Liposuction

Liposuction does not compensate for an unhealthy diet or a sedentary lifestyle.

Before Arrival:

Stay in Istanbul:

• A stay of 3–7 days is usually sufficient for:
