Frequently Asked Questions About BBL
1. What Is BBL (Brazilian Butt Lift)?
BBL (Brazilian Butt Lift) is a combined body contouring procedure in which your own fat tissue is used to reshape the buttocks, while at the same time reducing fat in areas such as the waist, back and abdomen.
In this procedure:
- Fat is removed from specific areas of the body using liposuction,
- The harvested fat is processed and prepared with special techniques,
- The prepared fat is injected into carefully selected areas of the buttocks to add volume, projection and shape.
The aim is not only to enlarge the buttocks, but to make the waist–hip ratio and the overall body silhouette more aesthetic and proportionate.
2. In Which Situations Is BBL Preferred?
BBL is usually considered in cases such as:
- A buttock area that looks flat, indented or poorly defined,
- Noticeable fat accumulation in the waist and back, while the buttocks lack volume,
- Desire to achieve proportions closer to an “hourglass” silhouette,
- Difficulty in clothing choices due to body proportions,
- Preference for a more natural look using your own fat instead of implants.
When planned together with liposuction of other areas, BBL can also be part of an overall full-body contouring strategy.
3. Who Is a Good Candidate for BBL?
Good candidates for BBL are those who:
- Are in overall good health and fit for surgery,
- Would benefit from improvement in buttock volume and shape,
- Have sufficient fat reserves in other body areas to allow for liposuction,
- Can comply with postoperative restrictions on sitting, sleeping positions and compression garment use,
- Have realistic expectations about the results and their longevity.
In very thin patients with insufficient fat stores, weight gain or alternative methods may need to be considered first.
In cases of uncontrolled chronic illnesses, clotting disorders or serious heart–lung problems, BBL is only planned after a detailed medical evaluation.
4. Preoperative Evaluation and Planning
Before BBL:
- The patient’s medical history, medications, chronic diseases and smoking status are reviewed,
- The waist, back, abdomen, flanks, hips and buttocks are examined in detail to assess how much fat can safely be harvested from which areas,
- A personalised plan is created based on body shape, skin elasticity and muscle structure,
- Expectations are discussed in depth; examples are used to explain possible buttock size, shape and waist indentation,
- Required blood tests, anaesthesia assessment and other investigations are completed,
- Instructions and adjustments are given regarding blood thinners, supplements and smoking in the preoperative period.
The aim is to clarify aesthetic expectations and lay the groundwork for a safe surgical process.
5. How Is BBL Performed?
(Liposuction and Fat Grafting)
BBL is a multi-step procedure performed under general anaesthesia.
Liposuction (Fat Harvesting) Phase:
- Through small incisions, liposuction is performed on selected areas such as the waist, back, abdomen, flanks (love handles), and when needed, the hips and thighs,
- In this phase, unwanted fat is removed and the circumferential body contour is reshaped.
Processing the Fat:
- The harvested fat is processed using special methods and separated from excess fluid and blood components,
- The goal is to use high-quality, viable fat cells.
Fat Injection into the Buttocks:
- The prepared fat is injected into the buttocks using fine cannulas in a layered and balanced way,
- The aim here is to achieve a natural curve, projection and rounder shape,
- For safety, the fat is injected in a controlled manner and into specific layers; the surgeon works with careful attention to vascular structures and tissue safety.
At the end of surgery, the incisions are closed and appropriate bandages and compression garments are applied to the treated areas.
6. Postoperative Process, Stay in Istanbul and Follow-Up Visits
After surgery, the patient is monitored in the recovery unit during the first hours. An overnight hospital stay is usually recommended.
First Days:
- Swelling, tenderness, bruising and a feeling of tightness in the liposuction areas and buttocks are normal,
- Painkillers and other prescribed medications are used regularly,
- Short walks are encouraged early on to support circulation.
Stay in Istanbul:
For patients coming from abroad or other cities:
-
A 7–10 day stay in Istanbul is generally recommended for:
- Early postoperative check-ups,
- Suture and wound care,
- Oedema and general condition monitoring.
During this time, flight planning, doctor visits and any needed early interventions can be carried out safely.
Return to Social Life:
- Light daily activities can typically start within a few days,
- For desk jobs or light work, return is usually planned 10–14 days after surgery,
- For jobs requiring prolonged sitting, using a special BBL pillow to offload pressure from the buttocks is recommended.
7. Sitting, Sleeping Positions and Compression Garment Use
After BBL, sitting and sleeping positions are critical for fat survival and shape maintenance:
- For the first 2–3 weeks, direct pressure on the buttocks should be avoided as much as possible.
When Sitting:
- Special BBL pillows or supports that keep the buttocks off the surface are used,
- Body weight is directed more onto the backs of the thighs and lower legs, not onto the buttocks.
Sleeping Position:
- In the first weeks, lying face down (prone) or on the side is usually preferred,
- Lying on the back may be restricted until the period your surgeon considers safe.
Compression Garment Use:
- A body compression garment is typically recommended for 4–6 weeks for areas treated with liposuction,
- It helps reduce swelling, supports skin adaptation and helps the new contour to settle,
- The duration and daily wear time of the garment are adjusted individually at follow-up visits.
8. Possible Risks and Complications of BBL
BBL is a procedure that requires special attention and experience. As with all surgical operations, certain risks may occur:
- Bleeding and infection,
- Irregularities or asymmetry in liposuction areas,
- Partial fat loss and contour differences in the grafted areas,
- Prolonged swelling, bruising or delayed healing,
- Wound dehiscence or more visible scars than expected,
- Changes in skin sensation (usually temporary, rarely permanent),
- Very rarely, serious complications such as fat embolism.
For this reason, BBL should be performed by experienced surgeons familiar with the relevant anatomy, in accordance with safety protocols.
All potential risks and complications are explained in detail before surgery, and the process is planned transparently.
9. BBL Results and Managing Expectations
The main goals of BBL are:
- To give the buttocks a natural, proportionate and aesthetic shape,
- To slim the waistline and create a more balanced body silhouette.
Early Phase:
- Due to swelling, the buttocks may look larger and feel firmer than the final result; this is temporary.
Within the First 3 Months:
- The body naturally absorbs a portion of the injected fat,
- The remaining fat cells, particularly in well-vascularised areas, can become permanently established.
It is not realistic to expect a result that “will never change”. Body type, weight fluctuations and lifestyle all influence the long-term outcome.
The most satisfying results are achieved in patients who:
- Have realistic expectations,
- Maintain a stable weight,
- Follow their surgeon’s postoperative recommendations consistently.
10. Planning for International / Out-of-City Patients
(Stay in Istanbul)
For patients travelling to Istanbul for BBL:
Before Surgery:
- When possible, an online pre-consultation can be arranged for general assessment and Q&A,
- Required tests may be performed either in the patient’s own country/city or after arrival in Istanbul.
Stay in Istanbul:
-
A stay of 7–10 days is usually a safe timeframe for:
- First postoperative check,
- Dressings and wound care,
- Compression garment adjustments,
- Early interventions if needed.
Fitness to fly—especially for long-haul flights—is assessed by the surgeon with special attention to clot and circulation risks.
After Returning Home:
- Recovery can be monitored particularly during the first month via online follow-ups,
- Patients may be asked to send photos and/or videos if questions or concerns arise.
Since each patient’s situation is unique, the final plan is always made on a strictly individual basis.
11. Frequently Asked Questions About BBL
Are all of the injected fat cells permanent after BBL?
No. A portion of the injected fat is naturally reabsorbed by the body. The remaining fat cells that establish a good blood supply can become permanent. Therefore, in the early period the buttocks may look larger than the final outcome.
When will I be able to sit comfortably?
For the first 2–3 weeks, you should avoid sitting directly on your buttocks for long periods. During this phase, special BBL pillows and specific sitting techniques are used. Afterwards, sitting time is gradually increased according to your surgeon’s recommendations.
What happens if I gain or lose weight after BBL?
The grafted fat cells behave like other fat cells in the body. Significant weight gain may cause the buttocks to enlarge more than expected, while significant weight loss may reduce volume. Maintaining a stable weight is therefore important.
When will the final results be visible?
In general, most swelling subsides within 3–6 months, and the shape of the buttocks and waist becomes much clearer. Photos taken during this period reflect the result more accurately.
Should I choose BBL instead of silicone implants?
Each method has its own advantages and limitations. In BBL, your own fat is used; with implants, a prosthesis is placed. The decision on which method is more appropriate depends on your body structure, expectations and medical evaluation, and should be made together with your surgeon.
